Summary: looking for locum CV Surgery coverage for the below dates to provide 24-hour call coverage across three different facilities (listed below).
* Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
* Holiday Must be 1.5x the regular rate. Per holiday hour, no OT
* On Call (Daily) $3,793 -$3996 Per 24 hours to include 8 hours gratis"
* 24 Hour Holiday Call Rate - Per 24 hours to include 8 hours gratis"
* Call Back $333-$381 Must match Hourly rate Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
* Holiday Call Back Must match Holiday rate Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Average Callback Hours : 8 hours
EMR:
* Clinic Setting: Cerner Mckesson Cardiology
* Hospital Setting: Paragon & CHS Proprietary EMR
* Patients per Day: 10-15
* Support Staff: typical in clinic & hospital resources
* Worksite Resources : typical in clinic & hospital resources
* Reason for Coverage: separation with current employed Surgeon
* Deal Breakers: Malpractice history, board and or license practice restrictions Should have board and license to practice, malpractice Is case by case
Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: Local & in close proximity to the hospital, use current GSA rates . In the event that the client preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market. travel/plan-book/per-diem- rates
Air Travel: $750/round trip cap.
Rental Cars: $65/day all in cap (including taxes and fees) regardless of car class.
Mileage: IRS Standard Rate

* Are both Thoracic and Cardiac required? yes
* How many hearts/ year is the program doing? 100 possible
* Is there a minimum number of heart surgeries that must be performed from a candidate proficiency perspective? Will review case by case but should have enough to maintain licensure and board
* Is the surgeon required to harvest veins or will a PA be present? Should be able to harvest own veins hospital may provide PA depending on volume
