No CRNA supervision. * Physician must work Monday Friday assignment blocks 7am - 3-pm; no call, no weekends Candidate Requirements : * Active IN State License or IMLC. * Board Status: Board Certified or Board Eligible. * Minimum Years Experience (if applicable): More than 2 years * Flu vaccination: Required or Need Medical or religious exemption. * Covid vaccination: Required or Need Medical or religious exemption. * Certifications: ACLS, BLS. * Do not submit a candidate without procedure logs (for the last 12 months). * Candidate must supply quality data from the past 12-months.
