Highlights
Community Physician Network is seeking an Internal Medicine physician or Endocrinologist with special interest in Type-II Diabetes management to join our Diabetes Care Clinic on the north side of Indianapolis. The physician in this role will join a multi-disciplinary team (NP, pharmacy, social work, Diabetes educator) in an outpatient setting. Ideal candidates will be collaborative and have a passion for educating patients to improve outcomes and quality of life.

·       Outpatient-only position

·       Monday-Friday day schedule

·       EPIC EMR

·       Excellent compensation and benefits (options for signing bonus, loan repayment, relocation)

·       Support of a multi-disciplinary team 

Licensure & Certifications

·       BE/BC in Internal Medicine or Endocrinology

·       Additional experience/certification in Diabetes care

·       Licensed or eligible for medical licensure in Indiana
