Physician Assistant Role Scope-
The Advanced Practice Provider (APP) will work in an ambulatory setting and will be responsible for the evaluation of urology patients presenting with urgent and/or elective kidney stones as well as management of patients following emergency care and surgery. The Stones APP will counsel patients on stone prevention, schedule patients for stone surgery, when appropriate, and coordinate care for testing for metabolic stones evaluations, collaborating with urologists, nephrologists and other healthcare professionals to provide comprehensive care for stones patients.
Our APPs initiate patient care meetings with the health care team to revise the plan of care as warranted. They participate in total quality management to positively affect patient and system outcomes to improve care. The position does allow for the possibility to cover nearby practice locations with Monday through Friday schedules with no call!
· Provides direct patient care in office consultations, including assessment, workups, triage, emergency work-ins, and other patient care activities. Position specializes in treating patients with kidney stones and other general urologic conditions
· Obtains and documents health history, performs and documents physical exam, and orders and interprets diagnostic tests
· Provides counseling to patients regarding diagnosis and expected course of treatment
· Collaborates with physician(s) to provide appropriate direct care interventions within scope of practice
· Collects, organizes, records, and discusses data as appropriate and uses sound clinical judgment based in best practice standards and evidenced based medicine
· Prescribes pharmaceuticals according to patient needs and conducts appropriate patient follow-up & outreach
· Ensures timely retrieval of laboratory and ancillary studies and diagnostic consultation reports
· Consults with appropriate internal and external staff to make appropriate referrals for acute, chronic and preventive care
· Coordinates patient care and visits with appropriate clinical and non-clinical staff

Travel is required between Hartford and Central Region

Qualifications
Advanced Practitioner Requirements
· Master’s degree in nursing
· National Certification in Advanced Practice and current CPR certification
· Current State of CT APRN license with prescriptive authority and current DEA number
· 2 years of experience (urology preferred)
· Excellent clinical management skills and proven ability to work collaboratively in a team environment and take lead when appropriate
· Positive, customer-focused approach, with commitment to providing excellent patient care
· Excellent written and verbal communication skills. Ability to communicate in other languages – highly desirable
· Ability to communicate technical information clearly to patients of all ages
· Highly organized with demonstrated ability to effectively prioritize competing tasks with full autonomy
· Knowledge of computer systems, including experience with medical software (Epic)
· Ability to adhere to HHC MG policies and standards of service as well as HIPAA requirements, OSHA regulations and Infection Control standards
· Communicates tactfully and effectively and demonstrates a commitment to superior patient care
· Establishes and maintains effective work relationships and an achievement-oriented attitude
· Displays courtesy and helpfulness when interacting with all internal and external customers
