High Fidelity Wraparound Family Support Partner

The High Fidelity Wraparound Family Support Partner will work as part of a Joint Planning Team to assist adolescents with emotional needs and their families. The Family Support Partner will provide education and limited direct support to help the adolescent and their family strengthen their natural support system.   The Family Support Partner (FSP) position is designed to provide intensive levels of peer support for families.   These positions are a distinctly different job than the HFW Facilitator, but they work closely with the Facilitator to support positive outcomes for the family. 

The Family Support Partner must have direct past personal experience with managing family needs, children/adolescents with behavioral and psychiatric needs, juvenile delinquency, substance abuse, or other needs that have prepared the Family Support Partner to be aware of available supports for families going though similar challenges and to have sufficient compassion, empathy and understanding for persons served and their family members.

Some responsibilities include:

  • Support the implementation of the 10 Principles of High Fidelity Wraparound.
  • Support the implementation of the 4 Phases of High Fidelity Wraparound.
  • Complete training in the High Fidelity Wraparound Model.
  • Work toward becoming credentialed as a Family Support Partner.
  • Provide direct support (in a limited manner) within the family-driven plan.
  • Ensure the Principles and Phase of the High Fidelity Wraparound process are delivered with the highest possible fidelity.
  • Work closely with team members to support positive outcomes for the adolescent and family.
  • Assist with linkage to natural and community supports.
  • Assist with crisis stabilization when needed.
  • Manage a caseload of up to 25 families
  • Works with other team members and mental health providers as needed. 
  • Ensures completion of all documentation requirements within determined time-frames consistent with CGRC procedures.
  • Other non-essential duties as specified by the immediate supervisor.
