The Patient Access Hospital RN is responsible for responding to hospice referrals and for performing admissions in contracted facilities. Act as a partner in care with physicians, case managers and other facility staff. Educate patients and families about the hospice benefit and services. Establish eligibility and facilitate the coordination of discharge planning with hospice and hospital staff. Provide initial physical assessment, admission to hospice as required. There will be occasional weekend and holiday coverage.
This RN position requires travel to hospitals within Ulster and Dutchess.

ESSENTIAL FUNCTIONS:
- Receives Intake information and provides assessment using the NHPCO assessment guidelines to assist in the determination of eligibility for routine or inpatient hospice services.
- Gathers data through the assessment of coping strategies, support systems and learning needs of patient/family members for the purpose of arranging hospice services.
- Communicates effectively with all members of the interdisciplinary team and facilitates discussions regarding end of life, patient status, comfort medications, discharge disposition etc.
- Coordinates with hospital and hospice personnel for the transfer from referral to admission status while facilitating a safe plan of care.
- Identifies and communicates with referring physicians, hospital nursing staff and case managers on an ongoing basis.
- Effectively documents all interactions with patients, families, and hospital staff prior to admission to hospice in both hospice and hospital EMRs.
- Initiate discussions regarding advanced directives and end of life care.
- Strong Customer Relations skills
- Can effectively communicate in English, both verbally (in person and by phone) and in writing.
- Ability to work independently.
- Demonstrate strong clinical assessment skills.
- Organization and attention to detail through effective methods
- Ability to prioritize and handle existing and new referrals throughout the day

- Working within assigned settings, including but not limited to Hudson Valley Hospice offices, hospitals, skilled nursing facilities, patient/family homes, physician offices and community agencies.
- Ability to carry up to twenty-five pounds of supplies and/or materials used to provide community education or into a patient’s home and transfer/position an average adult for care.
- Routine duties involve potential for direct exposure to blood or body fluids.
- Ability to work flexible hours when needed
- Valid NYS Driver's License and reliable transportation required.
- Graduate of an accredited nursing school with a Valid NYS RN license.
- Certification in Hospice and Palliative Care or receive certification within one year of employment.
- Minimum of two years clinical nursing experience. Previous hospice, home care and/or oncology experience preferred.
- Direct service experience in discharge planning, outreach and public relations preferred.
