BC Internal or Family MedicineStarting upon completion of credentialing to end of August Multiple outpatient clinics (Oakland, Berkeley, Oakland, Concord & San Leandro)Monday - Friday from 8:00am-5:00pm (no call)Average of 5 patients per dayManage Chronic disease states of geriatric populationMust work with care plans within group of: NP, IM Doc, PT, OT, dietician, and other specialties for each patientManage dialysis treatment set upCoordinate all care for these patients dailyEMR is E-Clinical worksBC IM/FM, CA license, DEA, BLS, Geriatric experience, PACE program or Team based practice experience and Covid Vaccine required
