NP/PA - Rural Health Initiative - New London

ThedaCare is seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive, coordinated care at ThedaCare Medical Center-New London Family Medicine Department in New London, Wisconsin.  

Details:

• 1.0 FTE 
• Best of both worlds seeing patients in their homes & via telehealth. Company car and all technology needed is provided.
• 36 contact hours
• 4 on days in-person in patient homes.  12 on days telehealth
• Registered Nurses, Community Health Workers (CHW)-will be in the patient home to facilitate the APC telehealth visits, Community Paramedic to run any rapid need interventions.
• BLS/ACLS/PALS Required
• ATLS Required
• Benefits available 

Previous experience preferred.
