WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking an experienced PRN Family, Internal and/or Geriatric Medicine Physician to join our PACE Program team at Franciscan Senior Health & Wellness in Indianapolis, IN to help meet the growing demands of our outpatient program. This is a part-time opportunity with potential to expand into full-time.

WHAT WE NEED:

Ideal candidate will have nursing home care and Family, Internal and/or Geriatric Medicine experience. Candidate would thrive in a fast paced, collaborative and creative environment. Physician will provide direct medical care as needed to participants in the home, the PACE center, and in other health care settings. Physician will collaborate with our PACE Medical Director and PACE Program interdisciplinary team to coordinate and deliver the highest quality of patient care. In the PACE model of care, our team may approve the services that patients need - you dont need to request insurance company approval for tests, procedures, and services for your patients because we serve as both a provider and a payor. Physician will practice with the support of our own network of social workers, dietitians, rehabilitation therapists, transportation service, home health services, and much more. These resources are located in the same clinic, for close collaboration and individualized care that truly addresses each patients expectations for a high-quality life.


WHAT YOU CAN EXPECT:

  • Practice Hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.

  • Traditional PRN Schedule

  • Call responsibilities based on provider staffing; minimum 1:3 rotation

  • Daily Duties:

    • Care planning with the PACE Program interdisciplinary team

    • Serve as the primary care physician for patients

    • Manage and round on PACE participants admitted to long term care facilities or acute care in participant facilities

    • Review medical histories and medical assessments

    • Implement plan of care with patient and family

    • May serve as collaborative physician to advanced practice providers

  • Outreach potential with our Lafayette location

  • Will work with skilled and long-term patients

  • Doctorate - Required

  • Physician License (MD/DO) - Upon Hire Required - and -

  • Board Certification in Specialty Discipline (or eligibility if appropriate) - Upon Hire Required

  • Basic Life Support Program (BLS) - American Heart Association - Upon Hire Required
