Northern IN - Close to several major metropolitan areas 90 minutes to Chicago 3 hours to Detroit & Indianapolis South Bend, home of Notre Dame and is only 25 minutes there is a train from South Bend to downtown Chicago Community Health Center is adding a Family Medicine physician to their team of 2 Family Medicine physicians, 4 Pediatricians and 3 APP's. Outpatient only Office Schedule: 5 days per week or can do a 4 day work week No clinic on weekends or holidays Productivity expectation: 25-30 patients per day Eligibility for National Health Service Corp student loan repayment plan (final decision made by federal government) Supervise certified nurse practitioner or other support staff Pager, Cell Phone provided Candidate requirements: Comfortable in a busy practice with diverse patient base Team work with medical staff and medical director to achieve established patient outcomes (quality of care). Must work well in team environment Bilingual (Spanish) a plus Must have interest in community medicine Offer: <
