The world-class cardiovascular medicine program at Cleveland Clinic is seeking a Heart Failure Cardiologist to join our established and highly respected health system within Cleveland Clinic Florida Cardiovascular Medicine. Cleveland Clinic Tradition Hospital is located in Port St. Lucie Florida, one of the fastest growing cities in the US. Cleveland Clinic Florida includes 5 hospitals and 33 outpatient facilities in Southeast Florida, throughout Broward, Palm Beach, Martin, St. Lucie and Indian River Counties.
We are seeking candidates interested in creating a program for heart failure at Cleveland Clinic Martin Health and Tradition Hospital. The focus of the position is three-fold: (1) creation of systems of care for high quality and efficient hospital care of heart failure patients, (2) population health management of outpatients with heart failure, and (3) appropriate identification and follow-up of patients needing advanced procedural treatments. Multidisciplinary coordination of care with primary care, cardiovascular specialists, and procedural teams will be a core component of the position. Electronic medical record population health analytics will also be an important aspect of the program.
A faculty appointment at a rank commensurate with experience is available.
