Dates/Times of Coverage: ASAP - Ongoing, Locums to Perm Schedule - 40 hours max Monday: 8:00am-5:30pm Tuesday- Thursday: 8:00am-8:00pm Friday: 8:00am-5:00pm EMR: E-Clinical Works Reason for coverage: Short 1FT FM MD Support Staff: nurses, PSR's and MA's Procedures: Annual Physicals, Sick/Well visits, Women's health, Immunizations and screenings, Treatment for chronic illness including diabetes, high blood pressure, and asthma, Office procedures Referrals to specialty services Other specialists on site: Pediatricians, behavioral health, other FM MD's and FNPs, DDS at nearby facility Daily Avg: 20-24 patients per day Avg per hour: FM- 2.5 per hour, Peds - 3 per hour FM: See All Ages Peds: would only see children Requirements: - Medicaid/Medicare patients accepted? Y o Provider required to have Medicaid number? Y - Any CPR/ additional certifications required? BLS Cert YES, BC YES - COVID vaccination required? N - Case-logs required?
