Growing together.The Clinical Performance Quality (CPQ) Pharmacy Operations team supports Optums Care Delivery Organizations by delivering patient-centered, clinically collaborative medication consultations to help people live healthier lives. Our team focuses on the design, execution, and delivery of telephonic and digital engagement strategies designed to close Medicare star measure gaps in care, including medication adherence, comprehensive medication reviews for MTM completion, statins therapy in patient with diabetes and cardiovascular disease. Our interdisciplinary service delivery team is comprised of Pharmacists, Pharmacy Technicians, and Care Coordinators.The Clinical Pharmacist will perform telephonic, patient-centered clinical consults focused on Medication Therapy Management (MTM), Care for Older Adults annual comprehensive medication reviews, conduct outreach, and consult to support statin utilization in patients with diabetes and cardiovascular disease. In addition, the clinical pharmacist will field clinical concerns escalated by the medication adherence call center agents via warm transfer. The clinical pharmacist will be required to meet or exceed established productivity and quality metrics and work a flexible schedule to support the hours of operation of the business.Youll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges.Primary Responsibilities:Conduct telephonic medication consultations that meet/exceed production and quality metricsComplete annual medication review of prescription and non-prescription medication with emphasis on patient assessment and counseling, safe and effective use of education, identification of potential drug interactions, and other related activitiesComplete pharmacotherapy assessment of patients claim, past medical history, and disease statesAssess patients for medication adherence barriers and develop a plan to maximize patients medication success through improving or maintaining adherence to medicationsDeliver high quality evidence-based therapeutic recommendations to providersDeliver a patient-centered, clinically collaborative medication consultationDevelop a post-consultation list of action items for the patient and/ or prescriberDocument clinical consultations into internal databasedParticipate in and contribute to staff meetings and other staff development opportunities and interdepartmental work groupsMaintain a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as authoritative information source on drugs and their utilization in therapyAssesses and interprets customer needs and requirementSolves moderately complex problems and/ or conducts moderately complex analysesProvides explanations and information to others on difficult issuesYoull be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Active, unrestricted pharmacist license in the state of primary residence and willing to obtain licensure in one or more states as per business requirements5+ years of pharmacy practice experience to include 2+ years of Clinical Pharmacy Practice experience1+ years of experience with Medicare STARs HEDIS Measures to include experience with Medication Therapy Management (MTM) and/or Care for Older Adults (COA) medication reviewWilling or ability to work between the hours of 9a-6pm CSTPreferred Qualifications:Doctor of Pharmacy degreePost Graduate Training (Pharmacy Residency and/or Fellowship)Board certification by the Board of Pharmacy Specialties (BPS)Certified Geriatric Pharmacist1+ years call center experienceBilingual in SpanishState License in Oregon, Tennessee, or Idaho*All employees working remotely will be required to adhere to UnitedHealth Groups Telecommuter Policy.California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Rhode Island, Washington, Washington, D.C. Residents Only:The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
