SANZIE HEALTHCARE SERVICES, INC.Job TitleLPN/RN Care Coordinator/Scheduler/RecruiterReports to:AdministratorJob SummaryResponsible for all areas of Operations: human resources, intakes, clients, caregivers, and schedulingAdministrativeUnderstand and adhere to established SHCS policies and procedures.She assisted with recycling, associate hiring orientations, and in-services-disciplinary actions relating to attendance, scheduling, requirements, and licenses.Oversees the new hire process for all branch hires; ensures all documentation is completed promptly and accurately. On-boards and trains new branch clinical and administrative employees.Complete wage and employment verifications of current and former employees.Interview, set up employee files, do bkg checks (Gchexs), and E-Verify prospective employees.Create caregiver folders in the computer and a physical file - ensuring all documentation is current.Respond to unemployment notices promptly.Maintain employee compensation, retention, and recognition programsEnsures HR policies and procedures are executed within the Agency.Provides reports and communicates information to key personnel.Other duties as assigned by direct supervisor or other members of ManagementAssist in training all branch new hires and current employees; may oversee or facilitate classes, including quarterly in-service meetings.Screens, Interviews, assists, and scheduling in hiring Home Care AidesReceive referrals and inquiries on programs and services offered by Sanzie Healthcare Services Inc.- complete the intake form and provide it to the Administration immediately.Provide documents for admission and supervisory visits to RN, as appropriate.Schedule supervisory visits and send out Quality Assurance Forms to Clients every two weeks or provide the said document to RN when supervisory visits are scheduled; evaluate reports when they are returned and address deficiencies, if anyFollow-up on referrals/inquiries of prospective clients for possible admissionSupervise caregivers and complete performance appraisals for caregivers at specified intervals.Enter, maintain, and correct client, employee, payroll, billing, and related accounting data into the computer.Act as the Agency's ongoing liaisonImplement the Agency's policies and procedures and ensure the Agency adheres to all State and Federal guidelines.Call clients every week to enquire about the quality of service.Attend community meetingsSend out in-service exercises to caregivers every month.Determine staffing needs recruit, select, hire, and orient prospective employees.Discipline and terminate personnel following company policiesCheck Axiscare and 4- tell us daily; validate by making sure Axiscare time logs are correct, dated, signed by both caregiver and client, and tasks completed per POC)Be available and collaborate with payroll personnel to ensure hours are correctAudit caregiver files at least every week to ensure complianceEnter data in Axis Care and 4-Tellius; maintain and ensure current and complete personnel records of caregivers and clients.Answer the telephone in the morning and evening, take inquiries or messages, and address issues using excellent telephone etiquette.Respond promptly and courteously to all callsProvide three weeks of on-call service as scheduledPerform clerical duties and other responsibilities, including but not limited to typing and distribution of office records, memoranda, calendars, minutes, agendas, etc., assigned by the Administrator.Protect the confidentiality of clients, caregivers, and AgencyCreate action lists and provide all completed and pending tasks to the Administrator dailyAccept Medicaid Waiver clients by completing EDWP formsSend out reports on Medicaid Waiver clients by the 5th of each monthEnsure that the reception area and common areas (reception and interview rooms) are always clean and trash is emptied dailyLearn the different functions of Axiscare and 4-Tellius and create systems (e.g., client and caregiver portals)Provides reports and communicates information to key personnel.Other duties as assigned by direct supervisor or other members of ManagementAssist in training all branch new hires and current employees; may oversee or facilitate classes, including quarterly in-service meetings.Screens, Interviews, assists, and scheduling in hiring Home Care AidesEnsures the appearance of the branch's open environment is professional: neat, clean, orderly, and generally free of clutterProvides customer service by answering employee questions accurately and promptly.Confirm all pre-hire steps have been completed, including completing pre-hire forms and maintaining the staff database (to include all requirements).Completes Forms I-9, verifies I-9 documentation, and maintains I-9 files.Review employee applications and required documents to ensure all forms are filled out and provide complete copies.Review all client information and plan of care upon receiving a new client. Put in an unassigned schedule to ensure monitoring and tracking of staffing progress.Schedule an appropriately skilled and unskilled person for the new client and input scheduling information into the database immediately after confirming with a caregiver.Check caregiver schedules to ensure they do not overlap with another client and monitor overtime, if applicable. Check to ensure the caregiver has the appropriate licensing and certifications for a case.Send information to the scheduled caregiver(s) via e-mail when assigning a new shift. Include any information relevant to the client and their location, if relevant. Texting may be used only with immediate staffing needs or confirmations.Confirm with the caregiver that they have received the information or ensure that Management or on-call confirms. Confirmations should be in writing or recorded if given verbally over the phone.Constantly remind caregivers about what is expected from them when relating to clients and agencies regarding dress, conduct, timeliness, etc. Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the departmentFollows all Medicaid and HIPAA regulations and requirements. Abides by all rules, policies, procedures, and standardsServes as the point of contact for new hires.Schedule replacement workers as needed and inform the family, client, POA, and necessary party of a change.Provides staffing for sick leave, vacation, and long-term leave.Communicate with clients and families about ongoing staffing needs. If shifts are canceled, contact the caregiver until confirmation is received by phone, text, and e-mail. Update schedules with a note reading that caregivers are aware of a change.
