Licensed Master Social Worker (LMSW)
Location: College Park, Ga OR Fayetteville, Ga.
Shift: Full Time or PRN/Part Time is Available.
Compensation: Negotiable
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our hospice team in Fayetteville, GA. The LMSW will play a key role in providing emotional, social, and practical support to patients and their families during end-of-life care. This position involves collaborating with an interdisciplinary team to ensure that each patient's holistic needs are met with dignity, respect, and empathy.
Key Responsibilities:
Patient and Family Support:
- Provide psychosocial assessments and counseling to patients and their families to help cope with end-of-life issues.
- Offer support with grief, loss, and bereavement counseling to family members before, during, and after the patient’s passing.
- Assist patients and families in making decisions related to end-of-life care, including advanced directives, funeral planning, and legacy building.
Care Coordination:
- Collaborate with the interdisciplinary team (nurses, physicians, chaplains, etc.) to develop and implement individualized care plans.
- Advocate for patient rights and ensure access to appropriate resources and services.
- Monitor and evaluate patient and family needs throughout the course of hospice care, adjusting care plans as needed.
Crisis Intervention and Support:
- Provide emotional and psychological support during times of crisis, including managing anxiety, depression, and fear.
- Facilitate family meetings to address concerns and ensure clear communication among the care team.
Education and Advocacy:
- Educate patients, families, and caregivers on the hospice care process, including available services, community resources, and coping strategies.
- Advocate for the patient’s wishes and ensure that care aligns with their values and preferences.
Documentation:
- Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and social work interventions.
- Ensure compliance with regulatory and accreditation standards related to social work practices in hospice care.
Team Collaboration:
- Participate in regular case conferences and interdisciplinary team meetings to provide updates on patient care and social work interventions.
- Contribute to team discussions and contribute to the development of care strategies that enhance the quality of life for patients and families.
Qualifications:
- Education: Master’s degree in Social Work (MSW) from an accredited program.
- Licensure: Current, unrestricted LMSW license in the state of Georgia.
- Experience: Minimum of 1-2 years of experience in social work, with preference given to those with hospice, palliative care, or end-of-life care experience.
- Skills and Abilities:
- Strong interpersonal and communication skills, with the ability to build rapport and trust with patients and families.
- Empathetic, patient-centered approach to care.
- Ability to manage complex emotional situations with professionalism and sensitivity.
- Knowledge of hospice and palliative care practices and the regulatory environment.
- Excellent organizational and time-management skills.
- Ability to work collaboratively in a multidisciplinary team.
Preferred Qualifications:
- Previous hospice or palliative care experience.
- Familiarity with local community resources and support services in the Fayetteville, GA area.
- Certification in hospice and palliative social work (SWHP-BC) is a plus.
