Home Care RN Job Summary
- The RN Case Manager is responsible for the clinical management of an assigned patient population.
- The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach.
- The RN Case Manager is accountable for achieving positive patient outcomes
- Supervises Home Health Aides and LPNs according to policy
- Reports daily to the Clinical Manager all pertinent patient information
- Reports/records information accurately
- Uses appropriate safety and infection control measures
Home Care RN Qualifications
- Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.
- Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.
- Valid North Carolina Drivers License and proof of auto liability insurance required.
