As the
Community
Support Services Program Manager you will beresponsible foroversite of the community support programs (IPR, HSL, PATH, Voc, and Wellness Center Programs) that provide services to persons with a serious mental illness, intellectual disability, or brain injury. The Community Support Services Program Manager is responsible for ensuring the programs run smoothly, meet participant needs, staff supervision, program monthly billing, and adhere to regulatory requirements including grant and other funder requirements. Additional responsibilities include:
Your Responsibilities Facilitates all aspects of the referral, intake and discharge process for HSL, Voc, and IPR programs. Ensure funding availability for clients prior to entrance to programs and monitor authorizations during the clients' program stay. Assigns and manages staff caseloads and maintains a waiting list of clients as necessary for HSL, Voc, and IPR. Admits clients and inputs insurance for Wellness Center Provides regular effective training and supervision to program staff and oversees the completion of job tasks and monthly productivity quotas. Completes productivity reports and billing for HSL, Voc, IPR, and Wellness Center Completes weekly and monthly KPI spreadsheets for all programs Conducts regular1:1 supervision meetings with team members of programs. Facilitates regular team meetings and ensures all staff complete required in-service training. Monitors completion of all client staffing reports, and clinical record requirements. Provides supervision for staff for crisis intervention and support needs of caseload. Participates in any surveys or audits of services and functions and assists in the implementation of change. Monitors the completion of intake, annual and discharge requirements by program staff. Effectively collaborates with other agencies and with other programs within Hillcrest and maintains positive relationships. Assists in the revision, development, and training of policies, procedures, forms and protocols. Knows and adheres to all licensing and certification standards, and grant requirements for programs. Conducts grant writing and preparation, data collection, adherence and reporting requirements, as assigned. What You Need
Bachelor's degree in social work or a human services field required. Minimum of two (2) years' experience in human services, preferably working with individuals with mental illness, intellectual disability or brain injury required. Experience in supervising What Makes You Stand Out
