Details Client NameMaximus - Manhattan Job TypeTravel OfferingNursing ProfessionRN SpecialtyHome Health Job ID29569130 Job TitleMAXIMUS : RN - Nurse Assessor - Manhattan - Uptown - NYIAP - Home Health Weekly Pay$2370.37 Shift Details ShiftDay - 8x5 - 09AM Scheduled Hours40 Job Order Details Start Date12/06/2024 End Date11/30/2029 Duration260 Week(s) Job Description Job Title: Registered Nurse - Home Health Job Specialty: Home Health Job Duration: 260 days Shift: Day - 5x8 - 09:00 AM - 05:00 PM - 40 Hours per week Guaranteed Hours: N/A Experience: Two (2) years of experience in a home setting within the last ten (10) years License: Current RN Licensure in New York State Certifications: BLS required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients- Experience in a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care setting- Experience conducting assessments that are similar to the UAS, such as OASIS- Ability to care for a minimum of two patients per week Job Description: - Perform initial in-home pre-evaluation of patients, including evaluation of health status, strengths, care needs, and preferences- Guide the development of individualized long-term care service plans- Review consumer medical documentation and health referral forms as relevant to the case- Enter evaluation data into electronic evaluation forms and transmit as required- Document any concerns, conflicting information, or other issues arising during the evaluation process- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans- Discuss health care options, needed supports, service vendor options, and waiver options with the consumer- Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan- Enhance communication and collaboration with interdisciplinary care team members to improve care coordination and facilitate service delivery- Collect quality review data and required documentation to support outcome measurements and record case notes into CRM- Identify opportunities for health promotion and illness prevention- Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population- Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template- Perform other duties as assigned by the Regional Nurse Manager or other project management Client Details CityManhattan StateNY Zip Code10038 Job Board DisclaimerBy applying for jobs on this website, you consent to receive daily messages from CYNET about assignments that match your profile.
