What is a Home Health Care Nursing Preceptor?

A Nurse Preceptor is a regional position that provides in-home clinical training and support to the local team. A Nurse Preceptor participates in educating clinicians on company-wide standards for patient care and documentation.

What does a Nursing Preceptor do?

A Nursing Preceptor is a regional role dedicated to providing clinical success in the local branches. A Nurse Preceptor maintains a modified patient caseload and travels to a patient’s home to administer home health services. A Preceptor will participate in co-visits with employees and assists primarily in the preceptorship of new employees. A Preceptor will also act as an educational mentor to established in-home field clinicians. Assignments are coordinated by the Regional VP of Operations and are on an as needed basis.

Some tasks include:

• Hosting Local Clinical Orientation
• Competency and Skill Sign-Off for Nurses
• Travel to All Branches in the Region for Support
• Acute & chronic disease state management
• Medication management
• Condition assessment
• Tailored, goal-oriented treatment plans
• Post-surgical rehabilitation

• Patient and caregiver education
• Pain management
• Wound care

What are the qualifications for a Nursing Preceptor?

• Advanced Understanding of Skilled Home Health Care including but not limited to:
  • Patient Driven Grouping Model (PDGM)
  • Face-to-Face Requirements
  • OASIS documentation proficiency
  • In-Home Rehospitalization Reduction Measures
  • Case Management Tasks
• Regional Travel as Requested
• Strong Clinical Skills (IV Therapy & Maintenance, Wound Care Treatment & Recommendations, Chest Tubes, etc.)
• Graduate of an approved school of nursing
• Unencumbered Registered Nurse Licensure

Weekday Schedule:

• Weekdays and occasional weekends as needed

Compensation Information:

• Salary
