• Start Date: Asap-ongoing
• Schedule: Mon-Fri 8:30am-5:30pm, 1 hr lunch
• Patient Volume: 25-35
• 100% Pediatric patients
• Cases: Extractions, space maintainers, fillings, crowns
• Credentialing Timeframe: 2-4 weeks

QUALIFICATIONS AND REQUIREMENTS:

• Pediatric Dentist or General Dentist who sees kids
• Active CO License
• Medicaid enrollment
