Job Description
We are seeking a qualified Case Management Supervisor interested in joining our Assertive Community Treatment (ACT) Navigator team at our Wilmington, Delaware location.
The Team Leader will provide managerial oversight to the ACT/ICM Team, manage the daily operation of the team in compliance with all state and local regulations and agency policies and procedures. The Team Leader will also provide ongoing review of participant’s placements, assessments of services and growth within the established system. Some additional responsibilities would include the following.
- Develop, supervise, implement and evaluate all program systems for the ACT/ICM Team.
- Provide leadership and overall direction to the team and staff.
- Provide administrative supervision of ACT/ICM team personnel.
- Will schedule regular supervisory and staff meetings, case conferences, and staff training and development.
- Coordinate, monitor and assure regulatory compliance for program service delivery.
- Review and assess all incoming participant referrals and assign to case managers; oversee the distribution and composition of staff caseload assignments; initiate redistribution and changes as necessary.
- Monitor and maintain an ongoing record of all participants referred directly to the ACT/ICM Team.
Qualifications:
- Master’s degree with major in psychology, counseling, social work, nursing, rehabilitation or related field and at least five (5) years of experience in the Mental Health field.
- Minimum of 3-4 years supervisory/management experience.
- Excellent communication skills.
- Detailed oriented and highly organized.
- Ability to work independently and as part of a focused team.
- Possess a valid driver's license with an acceptable driving record.
Scope:
- Requires sitting for long periods of time, frequent talking and listening.
- Requires the ability to lift, carry, fold, kneel, reach, bend, stack, stoop which may include direct intervention with participants.
- Must be available for local travel.
- Requires carrying a cell phone and being available for emergency situations.
- May be required to attend training to become a certified agency investigator.
