We are located at 4125 W Sample Rd, Coconut Creek, FL 33073 . #2024 QUALIFICATIONS: * Health - Ability to meet performance requirements. * Education, Experience and Skills: * Graduate from an approved school of nursing, College or University. * Must have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida. * Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current license. * Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF). * Must have knowledge of MDS 3.0 including Assessments, CAA's and Care Plans. * Conduct each Assessment and Care Plan with the appropriate participation of health professionals. * Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines. * Must be able to communicate well with any member of the Interdisciplinary Team. * Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record. PERFORMANCE REQUIREMENTS: * Must be able to sit, stand, bend, twist, lift and move intermittently throughout the day. * Must be able to lift, push, pull and otherwise move a minimum of 50 pounds. * Able to see and hear adequately in order to respond and interact with other staff and families. * Able to speak in clear, concise voice in order to communicate with patients, families, and other staff and disciplines. * Relate to and work with the elderly, disabled, emotionally upset and hostile Residents. * From time to time, you may be required to assume the duties of a floor nurse. SPECIFIC DUTIES AND RESPONSIBILITIES: * Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the patient. * Assure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process. * Conducts, completes or coordinates other disciplines in completion of the RAI and certifies the completion of the Assessment. * Sign and certify the CAA Summary Form. * Attend care planning and enter the care plan decision information. * Assist with month-end duties for closeout. * Perform other assignments as assigned.
