HIV Care Team Nurse
HIV Care Coordinator: Esperanza Health Center, a faith based primary health care clinic in Philadelphia, seeks a full time qualified and committed Registered Nurse for the HIV Care Team Nurse position. This position is responsible to coordinate and provide primary care services to HIV patients and other care team patients, in collaboration with the center's medical staff. Maintain individualized documentation/care plan for patients based on on-going assessment and identifies services needed. Provide ongoing assessment and triage of HIV and other care team patients who call or walk into the health center. Provide on-going patient education to empower patients to take an active role in managing their health. Work closely with the broader medical team to assure that patients receive needed services and referrals. Assist with HIV testing activities in the clinic. Offer spiritual care to patients in need. GENERAL RESPONSIBILITIES: * Works in harmonious partnership with health center staff and volunteers to further Esperanza's mission and vision, to accomplish common goals, and to coordinate various activities. * Adheres to policies and procedures of the health center and the regulations of governing agencies as they affect the department. * Promotes "safety" practices. * Effectively educates staff and others. * Seeks timely reconciliation with others following biblical principles when wronged or after doing wrong to someone else. * Favorably represents Christ and His mission at Esperanza within and outside the Health Center. * Adheres to dress code policy at all times. * Demonstrates habit of good attendance and punctuality by reporting for work whenever scheduled. * Participates in departmental meetings and general staff meetings. * Ensures the proper completion of work activities in a timely manner. * Supports and participates in Quality Improvement activities and projects. * Works effectively with all individuals in a non-discriminating manner. Education, Skills and Experience: * Licensed Registered Nurse. * Professional nursing experience in a hospital or community setting. * Experience working with patients with HIV/AIDS; demonstrated understanding of HIV/AIDS-related issues and standards of care preferred but not required. * Demonstrated knowledge of community health. * Ability to work independently as well as be a part of a multi-disciplinary care team. * Ability to integrate faith in work related tasks. * Excellent interpersonal and communication skills. * Bilingual in Spanish and English; ability to communicate professionally in written and verbal form. Spanish immersion training is available should you qualify for this position but are not fluent in Spanish. EHC will provide further information.
