Summary
Join our dynamic behavioral health telehub team as a Crisis Behavioral Health Therapist (BHT). In this vital role, you will assess, triage, document, and manage patients experiencing crises or requiring follow-up care for medical and behavioral health issues. Utilizing safety planning and suicide management techniques, along with psycho-education, you will empower patients through self-management and tailor behavioral change plans using best practices and evidence-based interventions. As a core member of the collaborative team, you'll work alongside emergency department, inpatient medical providers, psychiatrists, and primary care teams to ensure comprehensive mental and physical health care for patients.
Responsibilities
- (30%) Assess the clinical condition of patients referred by providers due to mental health crises and facilitate intervention processes to stabilize their condition. You will work closely with providers to make informed decisions regarding care levels and provide ongoing support to patients.
- (30%) Utilize brief assessment tools to identify patient needs and document treatment progress. You will monitor clinical symptoms and connect with patients to ensure follow-up care after crises stabilize.
- (20%) Assist in the development of program improvements, sharing best practices with team members, and curating patient education resources on mental health disorders and treatment options.
- (20%) Track follow-up and clinical outcomes using a registry, ensuring accurate documentation of patient interactions within the Electronic Health Record (EHR) system.
Other Duties and Responsibilities:
Your role will involve eliminating barriers to treatment, improving patient satisfaction, and increasing resource utilization. You'll handle referrals to external services, conduct family meetings, and build professional relationships with patients and families.
Certifications and Licensures
To be considered for this role, you must have a Master's degree in Social Work and hold licensure in the State of Michigan, or be actively pursuing full licensure within two years if a Limited License privilege is obtained.
Experience, Training, and Skills:
Knowledge of crisis therapy skills, including de-escalation, suicide prevention, and motivational interviewing is preferred. Strong interpersonal communication skills are essential, along with a commitment to ethical standards in behavioral health care.
