Description

The Director of Community Health Initiatives is responsible for leading the development of comprehensive programs and initiatives to support implementation of the Systems Community Health Improvement Plan as informed by the Community Health Needs Assessment.

The Director will work closely with stakeholders to address social determinants of health, identify and reduce disparities, fostering partnerships that advance health equity and improve health outcomes in the communities served by Emory Healthcare.

KEY RESPONSIBILITIES:

1.Strategic Planning and Implementation

Provide strategic leadership and direction for community health innovation initiatives within the Office of Health Equity, including the development and execution of new programs and interventions.

Oversees development of the Hospitals and Systems Community Health Needs Assessment and Improvement Plans.

Works collaboratively with hospital and physician leadership and community engagement teams to develop and assess innovative approaches to community health.

Develops and maintains strategic partnerships with community, government and other stakeholders to address health inequities in communities; including virtual and in-person outreach, education to community members and other community-based organizations.

Gathers and reviews data to inform program development, grants, and measurements.

Program Leadership and Management

Supervise the implementation and scaling of the Community Health Worker (CHW) program, ensuring alignment with best practices and community needs.

Oversee the planning, development, and management of community health programs, including budgeting, staffing, and resource allocation.

Monitor program performance and outcomes, making data-driven decisions to optimize effectiveness and impact.

Innovation and Continuous Improvement

Lead efforts to integrate technology, data analytics, and evidence-based practices into community health programs to enhance their effectiveness and reach.

Promote a culture of continuous improvement, regularly reviewing program outcomes and processes to identify areas for enhancement and scaling.

Prepare and present reports on program outcomes, challenges, and successes to internal leadership, community partners, and funders.

Ensure compliance with all regulatory and funding requirements related to community health programs.

MINIMUM QUALIFICATIONS:

Master of Public Health, Health Administration, Nursing, Community Health, or a related field.

A Doctorate in a relevant field is preferred.

Experience Minimum of 7 years of experience in community health program management, with a focus on innovation, health equity, and social determinants of health.

Proven experience in supervising and leading teams, particularly within community-based health programs.

Strong knowledge of and ability to support the measurement, assessment, and articulation of the impact of community health strategies.

Experience working with diverse populations and a strong commitment to advancing health equity.
