Hennepin Healthcare has a full-time (1.0 FTE) opportunity for a

Nurse Practitioner or Physician Assistant

to work in our downtown Internal Medicine Clinic. Clinic hours are M - F during the day.

This position is responsible for evaluation and treatment of patients with acute and chronic physical, mental health, and substance use conditions, as well as providing preventative care services and education to patients and families within scope of practice and in collaboration with the supervising/collaborating physician.

RESPONSIBILITIES

Provide direct primary care services to adult patients as part of the primary care interdisciplinary team in the ambulatory department

Perform initial and ongoing assessment of patients' physical, mental health, and substance use conditions

Obtain and evaluate health and medical history, perform physical exam, conduct preventive screening procedures based on age and history, identify medical and health risks

Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up

Formulate appropriate differential diagnosis based on history, and physical examination

Identify needs of the individual, family or community and develop a plan of care and/or education plan

Order, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and medications

Make appropriate referrals to other health professionals and community agencies

Reassess and modify the plan as necessary to achieve medical and health goals

Contribute to ongoing medical education teaching programs

Support new and existing programs which expand the organization’s role in the community

Participate in performance improvement activities

Adhere to productivity standards established by the department

Work closely with interdisciplinary teams to provide the best care for the population served

Respond timely to in-basket, forms, email, and paging communication regarding patient care

Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment

Perform other related duties but only after appropriate training
