We are seeking a compassionate and skilled Licensed Master Social Worker (LMSW) to join our healthcare team. The ideal candidate will collaborate with primary care providers to deliver therapy, utilizing evidence-based practices to support patients in improving their mental and physical health. As part of an integrated care team, the LMSW will assist in behavior modification, emotional support, and therapeutic interventions in a primary care setting.

Key Responsibilities:

Conduct assessments to evaluate patients' mental health and recommend appropriate therapeutic interventions.

Collaborate with primary care providers and staff to provide individual and family counseling, ensuring coordinated care.

Maintain confidentiality of patient records in accordance with healthcare regulations.

Guide patients and their families in developing coping strategies for emotional challenges, trauma, and life stressors.

Assist in navigating referrals for patients who need different levels of care.

Work alongside an integrated care team, including physicians and care managers, to deliver comprehensive mental health care.

Document patient progress and treatment plans accurately in the companys EMR system.

Act as an advocate for patients, helping resolve issues during crisis situations and ensuring appropriate services are provided.

Serve as the behavioral health expert for the care team, offering insights on mental health trends and treatment strategies.

Required Qualifications:

Current LMSW licensure in the state of Michigan.

Masters degree in Social Work or related field.

Previous experience working with diverse patient populations, including adolescents and geriatric patients, is preferred.

Experience in a primary care or integrated care setting is a plus.

Strong knowledge of human behavior, mental health disorders, and evidence-based therapeutic techniques.

Proficiency in using electronic health records (EHR) for documentation and patient management.

Ability to work collaboratively in a team-focused, patient-centered environment.

Excellent communication and interpersonal skills.

Physical Requirements:

Ability to lift up to 40 pounds.
