Job Details: Scheduled hours: PRN, variable hours, as needed
Job Description:
Provides education, coaching and exercise guidance to patients and their families regarding cardiac rehabilitation, including post procedure and post surgical recovery, and disease-specific symptoms and management. Participates in projects related to area of specialty. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the organization as well as the community. A clinically competent, registered nurse who delivers specialized care to the patient in the health care continuum utilizing the nursing process to assess patient health problems and needs, develop and implement treatment care plans, evaluate and implement appropriate interventions and maintain medical records. Competent and proficient in advising patients on health maintenance and disease prevention/maintenance, and/or provide case management. Effectively coordinates patient care with other health team members, while maintaining standards of professional nursing. Capable of adapting to a changing environment. Proactively identifies opportunities for service growth or changes in order to maximize program efficiency. Maintains knowledge of current Cardiac Rehab best practices, accrediting requirements, and changes in reimbursement. Responsible for managing program referrals in a timely and efficient manner, including bedside teaching, telephone follow up, and scheduling of evaluation post discharge and/or referral, etc.
Qualifications:
Current license as a registered nurse from the State of Iowa
