Sigma Healthcare team is looking for an experienced Social Worker who will help to oversee the actions of our current case managers that provide many services already to our patients.

Shift Information: Day Shift

Summary:

• The Social Worker will see patients as well as oversee the case manager's work as it pertains to social work, review and sign off on the progress notes that have been created. As well and provide insight into opportunities for our patients to gain better access to programs that will help them.

Responsibilities:

• The Social Worker is responsible for providing crisis intervention, psychotherapy, family therapy, and comprehensive case management services for assigned patients/residents and their families.
• This position may also lead or assist with the referral and admission process for assigned new referrals and provides technical guidance to doctors, other hospital staff, and outside agencies on the methods and procedures for handling a particular case, within State and Federal guidelines
• Position requires travel to and from other facilities within the region.
• Implement appropriate assessment-based treatment plans -Consults with team managers, statutory and voluntary agencies and patients' relatives to ensure compliance with patient's treatment plan.
• Documents patient therapy in accordance with both federal and state regulations as well as UNOS regulations.
• Assists clients/families with obtaining community assistance by referral to proper resources.
• Supports management of concurrent review, discharge planning processes, social services, home care and other post discharge needs

Qualifications:

• A current, valid licensed clinical social worker is REQUIRED; LBSW, LMSW, or LCSW
• One (1) year professional social work experience in a healthcare setting preferred. Such experience must have demonstrated ability to perform the most difficult and complex assignments.
