Job Title: Clinical Social Worker I Job Summary The Clinical Social Worker I collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. Essential Functions (Key Roles &Responsibilities)
1. P rovides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. Formulates biopsychosocial assessment, disposition and treatment plans.
2. C ollaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
3. M onitor, evaluate, and record client progress according to measurable goals described in treatment and care plan. Competencies/Knowledge, Skills & Abilities
1. D emonstrates basic foundational skills showing fundamental knowledge and a commitment to the application of professional values and ethics.
2. D emonstrates fundamental skills in formulation, assessment of risks, crisis intervention completion of psychosocial treatment plans, and appropriate documentation.
3. M ay need assistance and guidance in ensuring the patient needs, safety measures and concerns are brought forward. Emerging knowledge of internal and external resources.
4. D eveloping and demonstrating interpersonal collaboration in a medical setting - inpatient or outpatient.
5. M ay need coaching/guidance in this area. May provide some assistance and support with onboarding for new team members.
6. A ware of opportunities in department/service initiatives; participates in initiatives at an entity level. Uses data to measure progress.
7. D emonstrate fundamental skills under supervision. May need assistance in promoting collaboration and equity among health care team members, other colleagues, and organization to support, and enhance patient care.
