Our health system has four acute care hospitals and two specialty hospitals that together house 1,812 hospital beds.
Position Information:
LPG Palliative Careis seeking a physician with fellowship training and BE/BC in Hospice and Palliative Care Medicineto join their busy team. Currently, the team consists of 4 physicians and 13 APRNs. This exclusive physician group is team-oriented and quality-focused, and is currently seeking physicians interested in providing a mix of both inpatient and outpatient care Inpatient care will be provided at the hospital setting with potential SNF opportunities as well. Weekly call rotation is by phone 1:4 - 1:6. Candidates for this position will be interested in doing coordinated care, medication & chronic symptom management, end of life management, and goals of care discussions.
Our outpatient care currently being redesigned for full integration into some of our high-utilization clinics (specifically oncology and advanced heart failure). This is a great opportunity for someone with an interest in development, program growth and out of the box thinking with a high focus on exceptional patient care to join the team and assist with this strategic redesign process.
