PRN Hospice Medical Social Worker in Mandeville, Covington, and Slidell Louisiana Interim HealthCare Hospice provides compassionate end-of-life care, focusing on the physical, emotional, and spiritual well-being of patients and their families. We are seeking a dedicated PRN Hospice Medical Social Worker to join our team and make a meaningful difference during life’s most important moments. The PRN Hospice Medical Social Worker will provide psychosocial support to hospice patients and their families, addressing emotional, financial, and social needs. This position offers flexible hours to meet patient and family needs while ensuring compliance with all hospice care standards. Our Hospice Medical Social Workers enjoy some excellent benefits: Salary: $55,000-$60,000 annually 1:1 social worker-to-patient ratios where you impact outcomes Flexible assignments, autonomy and work-life balance Online training, growth and ability to expand skills Tuition discounts through Rasmussen University PTO, Holiday Pay, Medical/Dental/Vision & 401(k) Benefits As a Hospice Health Medical Social Worker, here’s a big-picture view of what you’ll do: Conducts an initial psychosocial assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient’s/family’s needs and the IDG plan of care. Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care. Identifies special needs related to religious or cultural diversity and how these impact the implementation of the IDG plan of care Provides counseling in response to identified anticipatory grief or pathological grief, if so qualified, or identifies alternative resources for such and provides direction to those resources. Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources, as well as provides follow-up and acts as a liaison when indicated. Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG. Participates in the development of the IDG plan of care and attends regularly scheduled IDG meetings. Assists the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end-of-life choices. A few must-haves for Home Health Medical Social Workers: Master’s degree in Social Work and active MSW license in Louisiana Minimum of 1 year of hospice or home health social work experience CPR certification, valid driver's license, active auto insurance policy, and reliable transportation Knowledge of state and federal home health regulations Strong interpersonal skills, good communicator, empathetic, compassionate and resourceful Why Work for Interim HealthCare?
