TITLE:
Physical Therapist
DEPARTMENT:
Rehabilitation
POSITION STATUS/HOURS:
Full-Time position (36 hours/week) Monday-Friday with on call weekend rotation (approximately every 7th weekend)
Department hours are Monday-Friday 7:30 a.m. – 5:30 p.m.
Schedule subject to meet the needs of GRHC. Focus of this position will be the Lancaster Outpatient/Clinic.

JOB SUMMARY:
The licensed Physical Therapist must meet all standards and requirements in accordance with the State of Wisconsin Department of Regulations and Licensing. The PT will organize and administer physical therapy services providing highly skilled evaluations and patient care primarily outpatient & inpatient cares. Home Health will also be performed as needed. PT’s assist with supervision of non-professional personnel in accordance with hospital policies and procedures. The physical therapist is part of a caring, dynamic team delivering PT and OT services to patients.
GENERAL RESPONSIBILITIES:
• Perform services in predominant outpatient/clinic setting. *Other services we offer and candidate may be assigned to assist with include: home health, inpatient, and swing-bed.
• Perform assessments designed to gather data relevant to the patient’s individual needs. Use medical record, evaluate cognitive status, evaluate sensory status, assess neuromuscular status; evaluate range of motion; perform manual muscle test; evaluate posture; analyze gait; evaluate functional activities; interpret physiologic response; evaluate balance/coordination; age-specific assessments
• Organize clinical care: verbalize priorities of patient/resident care for assigned patient care load; describe routine shift care; give rationale for clinical decisions; revise patient care priorities as needed; demonstrate ability to adapt responses to unexpected events; delegate procedural activities appropriately; complete assignments within specified time frame; demonstrate team building behaviors; participate in patient care conferences
• Implement the plan of care utilizing physical agents, perform manual techniques, perform joint mobility techniques, utilize exercise with equipment, utilize exercises without equipment, and provide adaptive equipment. Implements care plans with a PTA team approach & orthopedic cases.
• Demonstrate effective communication to patient/resident, co-workers, insurance companies, other disciplines within nursing home, hospital, and home health agencies
• Adhere to hospital safety policies and protocols
• Care for patients ages birth to geriatric
QUALIFICATIONS:
• Current Licensed Physical Therapist in the State of Wisconsin
• Graduate of a program in Physical Therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association
• Lymphedema certified or willing to become certified preferred.
• Possess a valid Wisconsin driver’s license
• Able to frequently transfer, lift, turn, or assist patient to or from bed, w/c, toilet or tubs. This requires the ability to push, pull, and lift 25 to 50 pounds unassisted
• Push/pull patients weighing 78 to 250 pounds in wheelchair or gerichairs. Reposition patients weighing 78 to 250 pounds in chairs and bed to perform therapy treatments, and patient comfort needs
