Shift: Full time, Evenings, Rotating weekends 40 hrs/week

Shift start: 1:30pm

Reports to the Laboratory Director with accountability to the Lab Support Services Supervisor, Lab Support Services Manager and the Chief Phlebotomist. Performs venipuncture, finger sticks, heel sticks and other specimen collection methods according to departmental and Northwest Community Hospital (NCH) policies and procedures for inpatients, hospital outpatients, client sites and/or nursing home clients.

1. Demonstrates the knowledge and skills necessary to assess and treat all age groups of patients from neonatal to geriatric; implements treatment based on the developmental stages of the patient. Performs venipuncture, finger sticks, heel sticks, POC testing and other specimen collection according to policy and procedure. Identifies correct patient before collecting specimen; labels all specimens accurately before leaving the patient. Completes phlebotomy procedure and specimen process for all timed specimen pick-ups, STATs and special lab orders within established time limits.

2. Identifies Medicare patients requesting services potentially not covered by the Medicare Plan according to compliance regulations; completes an Advance Beneficiary Notice (ABN) when indicated for non-covered services prior to providing service

3. Processes and documents verbal and written lab orders and standing orders in accordance with regulatory standards and hospital policy.

4. Correctly identifies patients before specimen collection and labels all specimens accurately before leaving patient bedside using the Rover device when appropriate. Provides compassionate patient care by employing hospital customer service methods and practices. Appropriately responds to questions and concerns of patients and their families. Maintains confidentiality of patient information.

5. Prepares, processes, handles, stores and packages specimens appropriately. Centrifuges specimens and transfers specimens into tubes or other designated containers as necessary. Accurately administers appropriate reagents for glucose and D-xylose tests. Performs point-of-care testing as required. Transports specimen to the laboratory. Instructs out-patients on timed testing procedures, distributes supplies, prepares and records collected specimens.

6. Performs order entry, logging and documentation as required. Maintains all records such as: logs, workload records, requisitions, etc. accurately and in a neat and organized manner. Utilizes hospital and laboratory information systems as needed to perform assigned duties. Performs basic troubleshooting of equipment and communicates problems to appropriate contacts.

7. Monitors supply quantities, restocks storeroom and outpatient areas. Notifies designated contacts of inventory needs.

8. Serves as a resource to other Phlebotomists and team members and assists in training.

9. Practices appropriate Standard Precautions as described in Infection Control and Prevention Activities. Adheres to all other departmental and hospital safety standards in order to maintain a safe patient care and working environment.

10. Adheres to all Northwest Community Hospital standards, policies and procedures.

Qualifications

1. High school diploma or GED equivalent required.

2. Basic knowledge of computer systems required.

3. Certification in approved school of phlebotomy or equivalent preferred.

4. Current CPR certification or must obtain within 6 months of employment.

5. ASCP certification or equivalent is preferred.

6. If responsible for collecting Nursing Homes, must possess a valid Illinois class “D” driver’s license and satisfy the annual Drivers Motor Vehicle verification review.

7. Interpersonal skills necessary to interact and communicate effectively with internal and external customers/clients.

8. Ability to simultaneously perform multiple tasks.

9. Ability to effectively work in various and stressful situations while displaying sensitivity and awareness of patients' physical and emotional needs.

10. Technical skills to efficiently utilize equipment and computers in performing assigned duties.

PHYSICAL REQUIREMENTS:

1. Moderate physical effort. Requires continuous standing, bending or walking. May require frequent lifting or moving of average weight material, or occasionally lifting or moving heavy weight material.

2. The flow of work and character duties involves normal mental and visual attention along with manual coordination much or all of the time.

WORKING CONDITIONS:

1. Conditions are acceptable. At least one disagreeable element or hazard is frequently present in the environment, i.e. exposure to infectious disease, while several disagreeable elements or hazards are occasionally present.
