Job Description
HeartWell Services, LLC is looking for a generous, outgoing, kind, compassionate, organized, and detailed nurse to join our staff. You will work within our Customized Community Supports and Family Living departments. Providing oversight, care, mentorship, instruction, and training to staff and contractors. You will create and follow up on nursing and medical emergency crisis plans, and complete annual nursing assessments, ensure adherence to plans, and offer input to the teams for persons served.
Position Summary - Provide coordination and supervision for the health care needs of individuals receiving services. To advocate for the person and ensure all health and safety needs are met with dignity. Provide mentorship, guidance, and training to employees/ contractors and provide direct support.
Position Duties and Responsibilities
- Contribute to the evaluation of the individual’s progress and provide input to the team concerning the healthcare of the individual.
- Create Nursing care plans and medical emergency crisis plans to meet the individuals' needs. Provide training on all plans to employees/contractors providing support.
- Compete annual (or as directed/ needed) nursing assessments.
- Utilize Therap as directed by state and agency standards determine.
- Review written and verbal reports/documentation on all medical and health-related needs/ findings and provide intervention or training when needed.
- Provide agency oversight of all medication management. Including pharmacy licensure.
- Provide after-hours on-call support.
- Work within the requirements and guidelines of the NM Nurse Practice Act.
- Conduct regular audits of medication storage and management in each location holding a pharmacy license.
- Track all medical appointments review physician consultation notes and provide follow-up on all recommendations.
- Provide monthly reviews and updates of all MARs.
- Respond to employee and contractor contact regarding any illness or injury of an individual. Provide direction and guidance through prudent nursing judgment. This may include a face-to-face nursing assessment at the time of the report.
- Provide emotional support and feedback to the individual and their family if applicable.
- Evaluate employee/contractor competency of individual diagnosis, health plans, and all recent changes in conditions. Provide formal and informal feedback and training to the employee/ contractor.
- Complete all required training and work within all HeartWell Policies & Procedures.
- Maintain regular communication with the Service Coordinators, Program Director, Medical Assistant, and HR on all appropriate related matters.
- Current Nurse licensure
- Valid Driver’s License
- Safe insured/ registered vehicle
