GENERAL DESCRIPTION:

The Prevention Case Manager will work with patients newly diagnosed with HIV and high-risk populations at risk of HIV. The position will ensure coordination of care for newly diagnosed individuals and will help those that receive PrEP.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Provide case management and care coordination for newly diagnosed individuals, ensuring access to medical care within 30 days of diagnosis
  • Provide support to patients by linking them to enabling and social services and follow-up to ensure linkage occurred
  • Provide ongoing follow-up and patient engagement activities
  • Provide support to patients that increases treatment and prevention adherence rates
  • Provide patient-centered activities that focus on the patient’s psychosocial needs particularly as they relate to being newly diagnosed or at risk of acquiring HIV
  • Supporting the education of team members on HIV Prevention efforts
  • Provide patients with appropriate support and education related to diagnosis and/or prevention
Strictly adheres to all Evara Health, HIPAA, HRSA, State, Federal and accreditation agency rules and regulations

EDUCATION AND EXPERIENCE:

  • Graduate of an accredited RN Program
  • Required: Minimum two (2) years as an RN in a healthcare setting with some supervisory experience
  • Required: Minimum two (2) years working with pediatric population

LICENSES, CERTIFICATION OR REGISTRATION:

  • Active and unencumbered RN License in the State of Florida
  • Current BLS Certification

ESSENTIAL PHYSICAL SKILLS:

  • Able to walk, stand, talk, use hands to finger, handle or feel, reach with hands and arms, climb and descend steps, carry or lift up to 25 lbs., stoop, kneel, crouch and/or crawl.
  • Able to read up close or at a distance, identify and distinguish colors, have peripheral vision (up, down, left, right, while eyes are fixed on a given point), and have depth perception (3‑dimensional vision, judge distance and spatial relationships).
  • Able to communicate, verbally and in writing with patients, vendors and/or co-workers.
