Must be a Licensed Social Worker from an accredited school of Social Work. Bachelor’s Degree from an accredited college or university in a healthcare related field. Master’s Degree is preferable.

Primary Function and Relationship to Total Organization: Plans, organizes, directs, defines, and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines. Provides case management, education and discharge planning. Performs community and education activities.
ROLE DESCRIPTION: The Social Worker has an important role at HCMH. We are the advocate for the patients as well as their family members. Our efforts are focused on improving the overall wellness for Hcmh patients emotionally, physically and spiritually. We are involved in a variety of services from marketing, engaging patients in social activities, assessing patient needs, testing, tracking patient satisfaction, and providing spiritual guidance, comfort and reassurance. We serve as the connection between the patient and all those serving the patient from nurses, physicians, and other care providers.

Essential Duties and Responsibilities:
- Conducts individual, group, and family sessions for patients as indicated by treatment plan.
- Facilitates identified support groups to meet the needs of patients/families as indicated by chart documentation.
- Participates in the development of master treatment plans and updates.
- Participates in weekly care plans and executing treatment regimes.
- Contacts community resources to ensure continuity of care as evidenced by chart notes
- Plans and arranges admission to special care facilities as indicated by physician’s request.
- Completes necessary psych. testing on admit, discharge and PRN.
- Conducts patient satisfaction study on or with referral sources.
- Interviews patients and families to obtain social histories and assures completion of psychosocial on each admission within 72 hours of admission.
- Completes weekly documentation of family and NH contacts and discharge planning.
- Organizes and facilitates public relation and community educational in-services.
Experience/Education:
Qualifications:
- Must be a Licensed Social Worker from an accredited school of Social Work. Bachelor’s Degree from an accredited college or university in a healthcare related field. Master’s Degree is preferable.
- Minimum 2-year experience patient centered care
- Current certification in Basic Cardiac Life Support required.
- Able to lead & facilitate meetings across the enterprise and across diverse audiences.
- Advanced data analysis and interpretation skills are needed to lead the service improvement effort and to create the credibility needed for interaction with hospital leaders and faculty.
- Ability to communicate effectively verbally and in writing with all levels of personnel, physicians, admin, leaders, patients, visitors and the community.
