JOB DESCRIPTION Title: CILA QIDP Reports to: Executive Director Department: Administration Full Time Exempt Benefits: Eligible ESSENTIAL FUNCTION The purpose of this position is to oversee the planning, development, coordination, monitoring, evaluation and compliance of all program services for each individual residing in the CILAs and all support staff. The CILA QIDP will function as the primary Qualified Intellectual Disabilities Professional (QIDP) in accordance with the established state and federal regulations affecting Community Integrated Living Arrangements serving adults with intellectual developmental disabilities. Carry out the duties in a manner that reflects and upholds the mission and philosophy of Covenant Ability Network of Illinois, CANIL. The position requires that consistent guidance and training be extended to all IDT members responsible for the comprehensive implementation of each individual's active treatment program. The position ensures that individuals served have services provided to them in a manner that ensures their privacy, safety, supervision, comfort, and well-being. The CILA QIDP seeks out and implements opportunities for individuals served to experience growth, social activities, community inclusion, and spiritual expression. QUALIFICATIONS The Americans with Disabilities Act prohibits discrimination against any qualified individual with a disability. To be qualified for this position an individual must meet the following standards and be able to perform the essential functions of the job. Must be legally eligible to work in the United States Minimum: Bachelor's Degree in human services field Prefer 2 years of related experience working in similar environment. Prior supervisory experience is preferred. Must be proficient in Microsoft Office Suite applications, including Word, Excel and Outlook. Must pass pre-employment physical and drug test within 3 days of contingent offer of employment. Must pass an Illinois state criminal background check with no disqualifying conditions. Must pass TB test within 30 days of employment. Must be able to pass the TABE test at 80%. Must have a valid Illinois driver's license with a good driving record to maintain insurability in accordance with MVR. Must be QIDP certified within 6 months of employment. Must be DSP certified within 120 days of employment RESPONSIBILITIES Works in coordination with the DSP Lead to ensure appropriate staffing coverage are in place for all shifts in the CILA. Works with Executive Director and Finance and Personnel Manager to develop annual program budget for the CILA, which includes the development and monitoring of staffing patterns. Meets monthly with the Finance and Personnel Manager to monitor budget and makes adjustments when necessary. Completes 30-day staffings, monthly progress reports, semi-annual reviews and annual reviews within required timelines. Is a member of and participates in quarterly meetings. Incorporates all assessed needs into the individual interdisciplinary planning process. Plans for, facilitates, records the Minutes and obtains necessary signatures for each individual's interdisciplinary team staffing and or meeting. Ensures the involvement of each individual and their guardians in all aspects of their program development 21 days following the IST meeting, the ISP, training objectives, measure for progress, and data collection method will be completed and distributed to team members. Provide on-going training and documentation of training, to all staff responsible for the implementation of new or revised individual training objectives and support plans. Observes and provides feedback to Direct Support Professionals on the implementation of individual service plans, and behavioral support plans to ensure they are being carried out as written and that interactions with individuals reflect the philosophy of CANIL. Provides accurate and timely documentation of the individual’s well-being, progress, and changes of circumstances or conditions. Completion of monthly summaries of each client’s progress data Revise individual program objectives when behavioral criteria have been met, when no significant progress has been made, or at the recommendation of the IDT and Behavior Management Committee Promote a clean, sanitary, and pleasant personal appearance for effective role modeling to the individuals served. Facilitate, coordinate, and document Minutes and retain a signature of attendance sheet at regular bi-monthly staff meetings and training in-services Observe and report needed repairs and equipment in need of service. Maintain regular communication with family members and other interdisciplinary team members. Provide service coordination with each individuals’ vocational training program and the documentation of such efforts. Ensure individuals are educated on their rights. Quarterly, present information and data to the Behavior Management Committee, Psychotropic Medication Review, Human Rights Committee, other agency meetings and meetings with the Executive Director/Administrator. Provide supervision for each individual according to their plan, consistently involving them in activities, and establishing routines. Facilitate the admission, orientation, discharge and transfer of all individuals receiving CILA services. Follow the rotation and assume the duties of the Manager-On-Call. Conduct employee performance reviews at least annually. Provide and document coaching, teaching, and corrective discipline for staff under direct supervision. Responsible for scheduling and conducting interviews with potential employees. Checks references and extends offers of employment to candidates. Supervise Direct Service Professionals and Lead DSP working in the CILA Assist in the planning of and attend, all special events, family meetings and Board meetings as requested by Executive Director. In coordination with the Executive Director/Administrator is an active participant in state surveys, licensing reviews and the development of and implementation of any corrective action plan. Ensure drills occur as required (fire, tornado, emergency evacuation drills). Help coordinate annual training for disaster drills. Have polite and cooperative interactions with each individual, their family members, visitors, co-workers, supervisors, IDT members, neighbors and vendors. Perform other duties applicable to individuals served and the needs of CANIL as assigned by the Executive Director. Safely operate the vehicle when transporting residents to and from work, on activities, on routine appointments and whenever ever operating a vehicle during work time. ENVIRONMENTAL CONDITIONS Is subject to flexible hours including split shifts or varied hour including mornings, afternoons, evenings, and weekends. Sits, stands, walks, bends, lifts, and moves intermittently during working hours. Is subject to frequent problem-solving activities. Has regular contact with staff, individuals, family members, and the general public. May be subject to hostile and emotionally upset individuals, staff, family members and the general public. May be exposed to infectious diseases, odors, dust, etc. throughout the day. Work location may change with little or no notice. May be exposed to pets. May be experience frequent interruptions and varying levels of noise. On a temporary basis may work in other positions as needs dictate, including evenings, holidays and weekends.
