This position is full-time 36 hours per week on straight nights, 7p-7:30a. It will be located at the UH Management Services Center in Shaker Heights. This is not a remote position.
A Brief Overview
Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management.
What You Will Do Coordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resources Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations Displays leadership that influences optimal clinical and operational outcomes Delivering exceptional customer service while facilitating communication across multiple disciplines Possess and obtain working knowledge of external and inter-facility protocols and standards of practice Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach Responsible for promoting a Best Place to Work Culture and positive and professional work environment Radio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care Transport Dispatch of UH Critical Care Transport ambulances Additional Responsibilities
Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications: Education
(BSN) Bachelor's Degree in Nursing (Preferred) Work Experience
5+ years of acute care hospital in-patient nursing (Required) and ICU or ED experience (Preferred) Knowledge, Skills, & Abilities
Excellent interpersonal and communication skills both written and oral (Required proficiency) Strong organizational and multi-tasking skills (Required proficiency) Demonstrates a professional demeanor when interacting with patients and customers (Required proficiency) Strong computer skills including all MS platforms (Required proficiency) Excellent ability to perform triage in order to determine appropriate facility, bed type, service, etc (Required proficiency) Licenses and Certifications
Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire) Physical Demands
