Description
SHIFT : M-F some weekends / PAY RATE: Negotiable
Position Purpose/Summary: The Social Workers has responsibility for the social and
psychological well being of residents and families.
Essential Functions (Reasonable accommodations may be made to enable individuals with
disabilities to perform the essential functions):
1. Conducts in-service dealing with residents' rights, advanced directives, and psychological
needs of aging as necessary.
2. Acts as a liaison for residents and families.
3. Maintains adequate charting records as required by CMS, NCDHHS, DFS and DMA. This
includes a social history, social assessment, resident care plan, discharge plan and progress
notes.
4. Participates in the interdisciplinary care plan process and meetings.
5. Assists families with medical review procedures and appeal processes.
6. Attends workshops to provide continued learning experience related to geriatrics.
7. Serves as a member of the following committees: Interdisciplinary Care Plan and Quality
Assurance and Performance Improvement Committee.
8. Helps coordinate Resident's Council meetings.
9. Provides information to residents and family, Medicare, Medicaid and other welfare agencies
concerning social services or needs of the resident.
10. Compiles information with community health and Social Services available and keeps an up-todate resource file on these community services.
11. Supervises all approved college students in term/field work as requested.
12. Maintains contact with family members, involving them in the resident's total care, providing
them with non-medical reports when required or appropriate.
13. Functions as a resident advocate within the facility or home.
14. Participates in educational programs as required.
15. Reflects LSC's mission, vision, values and customer service standards in daily interactions.
16. Completes other assigned duties as may be necessary for the orderly operation of the facility.

Qualifications
Qualifications (Minimum Job Requirements):
Education: 4 year degree in social work or related field; must meet minimum requirements
specified by the State/Federal regulations
Experience: 1 year experience in long term care setting preferred

Specialized knowledge, licenses, etc:
