Job Title: Hospice RN Case Manager

Location: Twin Cities Metro Area

Schedule: Full-Time

Compensation: $90,000 - $130,000 + Sign On Bonus!

Position Summary:

Nurses looking for autonomy and variety should consider practicing in hospice care, the fastest growing segment of the healthcare industry, as a Hospice RN Case Manager. Provide care for clients in their home environments, building interpersonal connections with those you care for, by helping clients achieve their goals to remain comfortable and dignified at home.



Responsibilities:

  • Perform initial client assessments and create their plan of care at home
  • Complete OASIS timepoint visits: admissions, re-certifications, and discharges of clients
  • Coordinate interdisciplinary services and obtain physician orders for appropriate care
  • Administer nursing care and education to enable clients' comfort and dignity
  • Communicate continuously with patients, families, and other care team members


Qualifications:

  • Licensed as a Registered Nurse / RN in the state of Minnesota
  • Proficiency with Electronic Medical Records (EMR) software is necessary
  • Prior Nursing experience in a TCU, Hospice, or Home Health setting
  • Valid Driver's License and reliable vehicle with current insurance
  • Excellent at planning, organizing, and communicating
  • Demonstrated ability to work autonomously
