Job Summary: The Licensed Social Worker (LSW) is responsible for providing social services to individuals, families, or communities to enhance well-being, address challenges, and connect clients with resources. LSWs conduct assessments, provide counseling, advocate for clients, and assist in navigating systems of care. They work within ethical, legal, and organizational guidelines to promote the overall mental, emotional, and social health of clients. Key Responsibilities: Assessment and Evaluation: Conduct comprehensive assessments of clients' mental, emotional, social, and financial needs. Identify strengths, challenges, and goals for clients through interviews, observations, and case history reviews. Use standardized tools and frameworks to assess mental health conditions, family dynamics, environmental stressors, and social factors affecting the client’s well-being. Document assessment findings and develop individualized service plans based on client needs. Counseling and Therapy: Provide individual, family, or group counseling to address mental health, behavioral, and emotional issues. Utilize various therapeutic techniques, such as cognitive-behavioral therapy (CBT), solution-focused therapy, or trauma-informed care, depending on the client’s needs. Address issues such as anxiety, depression, relationship problems, grief, trauma, substance abuse, and life transitions. Support clients in setting goals and developing strategies to overcome obstacles, build coping skills, and improve quality of life. Resource and Service Coordination: Assist clients in accessing community resources, including housing, healthcare, financial assistance, legal support, and childcare. Connect clients with mental health services, addiction treatment, or other forms of care as needed. Coordinate care and services with other professionals, including doctors, psychologists, nurses, and case managers, to ensure holistic and comprehensive care. Advocacy and Support: Advocate for clients in legal, healthcare, or social services settings, helping them navigate complex systems such as welfare, insurance, or public assistance programs. Educate clients about their rights and available resources to empower them to make informed decisions. Work to remove barriers to services or advocate for systemic changes that benefit vulnerable populations. Support clients in crisis situations by providing immediate intervention, referrals, and guidance on managing urgent needs. Documentation and Recordkeeping: Maintain accurate and up-to-date case records, treatment plans, and progress notes in compliance with ethical, legal, and regulatory standards. Ensure that all documentation adheres to confidentiality laws, including HIPAA, and is accurate for purposes of billing, reporting, and continuity of care. Prepare reports for agencies, healthcare providers, and other involved parties as required. Collaboration and Teamwork: Collaborate with a multidisciplinary team of healthcare providers, school staff, community organizations, or other social service professionals to coordinate services and improve outcomes for clients. Participate in case conferences, meetings, and consultations to share relevant information and improve service delivery. Provide consultation and support to colleagues, other social workers, or staff members on complex cases. Crisis Intervention: Respond to clients in crisis, providing immediate intervention and de-escalation techniques to stabilize their emotional or psychological state. Assess risk for harm to self or others and intervene as needed, including coordinating emergency services or referrals to inpatient care. Assist in managing difficult situations such as child abuse, domestic violence, or suicidal ideation. Professional Development: Engage in ongoing professional education to stay current with the latest social work practices, ethical guidelines, and legal regulations. Attend workshops, training, and conferences to enhance clinical skills, stay informed about new research, and improve service delivery. Maintain necessary licensure and credentials, ensuring compliance with continuing education requirements and state regulations.
