TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in White Plains, New York, 10601 About The Position Job Title: Registered Nurse (RN) - Care Manager Job Specialty: Case Management (Utilization Review) Job Duration: 13 weeks Shift: Days - 10-hour shifts, 8 AM - 6 PM Guaranteed Hours: 38 hours per week Experience: Minimum of 2 years License: RN license required Certifications: BLS, COVID Vaccine Required Must-Have: - Discharge experience required - PRI Certification (to be obtained within 3 months of initial employment) - Screen certification strongly preferred - Care Management certification preferred (e.g., CMC, CCM) - 3 years of clinical ED, critical care, or med/surg experience preferred - Previous case management experience preferred - Knowledge of healthcare financial, regulatory, and payer issues preferred - Knowledge of state, local, and federal programs strongly preferred - Experience in the use of Milliman/InterQual criteria - Essential computer skills Job Description: - Conduct comprehensive initial assessments on admission according to departmental policy, using established tools to identify high-risk indicators. - Provide ongoing reassessment of patient needs throughout the hospital course. - Perform utilization review activities as per department policy. - Engage in multidisciplinary care coordination within and between hospital levels of care. - Execute all transitional planning activities, including patient and caregiver education, arrangement of aftercare, commercial payer involvement, and addressing high utilizer patient root causes. - Communicate and collaborate all relevant patient information with appropriate healthcare team members. - Ensure the achievement of department goals, including length of stay reduction, readmission prevention, denial reduction, and satisfaction improvement. - Document following department requirements. - Participate in performance improvement activities as needed. - Collaborate with healthcare professionals to evaluate needs and safe discharge of geriatric patients, including, but not limited to, inpatients, emergency department, and surgical outpatients. - Perform other related duties as assigned. 26374436EXPPLAT Job Requirements Required for Onboarding ACLS - AHA BLS BLS - AHA Core Competency Pharmacology Skills Checklist Specialty Exam About Cynet Health Your travel nursing adventure begins here.
