Job Description & Requirements Pharmacist - Acute - (Pharma / Pharm) StartDate: ASAP Available Shifts: 11.5 N Pay Rate: $1638.08 - $1887.35 Pharmacist needed asap 8:30 P-8A / 13 Weeks Type of equipment – PyxisES, DoseEdge, KitCheck, Vigilanz, OnBase, Medex EMR - Meditech Must have: candidates without these skills will not be considered for the role. · Vancomycin, aminoglycoside, warfarin, heparin dosing · Medication order verifications · IV product verifications · Renal Dose adjustments, IV to PO Conversions REQUIRED CERTIFICATIONS: · Texas Pharmacist License · IV Certificate Preferred or nice to have: candidates with these skills will be considered first. · Critical Care Experience · NICU Experience · Pharmacy Residency · Board Certification List typical procedures performed on unit(s): · Medication Order Verification · Medication Dosing Consults (Vancomycin, Aminoglycosides, Warfarin, enoxaparin, acetylcysteine) · Antimicrobial stewardship, opioid stewardship · Working through automatic protocols such as IV to PO conversations, Renal Dosing Best personality Fit: · Team player · Positive attitude · Collaborative Preferred Qualifications Preferred or nice to have: candidates with these skills will be considered first. · Critical Care Experience · NICU Experience · Pharmacy Residency · Board Certification Facility Location Kingwood is heavily wooded with tall pine trees and fragrant magnolias.
