• Full-Time/ Permanent Role
• Mon-Fri 8am- 5pm - some shared call
• 100% outpatient
• Duties include; conduct home and face to face visits in SNF & ALFs within a 20 mile radius providing end of life care
• NC license in hand or in the process
• Ideal candidate will have 2 years of Hospice or Palliative care experience
