Nursing Home Social WorkerAmsterdam Nursing Home is actively seeking an experienced Social Worker for our Skilled Nursing Facility in the heart of New York City.Must have social work nursing home experienceDUTIES:Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needsIdentifying individual social and emotional needsDeveloping and maintaining individualized care plansParticipating in the planning of the resident's admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentREQUIREMENTS:Bachelor's Degree in Human Services; Psychology or Social WorkLong Term Care/ Subacute Experience RequiredRates:$35-$40 / HRABOUT US:With Columbia University and the Cathedral of St. John the Divine among its neighbors, Amsterdam House is located on Amsterdam Avenue at 112 street in New York City.
