Overview
You are the expert patients rely on to understand their medications and how to safely and properly use them. You make life better for patients by working in collaboration with other ambulatory clinic staff and providers to ensure proper medication management, patient education and optimization of drug therapy regimens. Responsibilities
CLINICAL COMPETENCE Optimal patient care outcomes from drug therapy are ensured through clinical pharmacy services. Such services are age appropriate, performed when needed through direct contact with the patient, and in direct association with or in support of health care providers. Participates in the development, implementation and measurement of outcomes of disease state management initiatives in the ambulatory setting. Works as part of a multidisciplinary team to provide a variety of key services in guiding the member/patient through the continuum of care while serving as an on-site resource to providers and staff. Key services include disease management, health assessment, transition planning and education. Assures appropriate documentation of drug therapy interventions in patient’s medical record. Provides ongoing education on medications to patients, providers and clinical staff. WORKFLOW PRODUCTIVITY Efficiently provided services are ensured. Coordinates with provider, clinical resource coordinator/resource specialist and other clinic staff to utilize these resources most effectively and efficiently. Provides timely interventions to optimize patient flow within the clinic setting. Completes fee tickets appropriately and accurately in a timely manner. PROFESSIONAL DEVELOPMENT Continuous sharing and application of drug therapy knowledge and skills are coordinated to improve the quality of pharmaceutical care provided. Works with other healthcare providers to ensure medication selection that is evidence-based and cost effective. Prescribes, adjusts dosage and monitors medication effectiveness per medical staff approved treatment protocols. Obtains a minimum of 30 hours of continuing education relevant to practice setting. COMMUNICATION
: Appropriate information, verbal or written, is effectively transmitted or received to enhance communication with customers. Communications are tailored to meet age/educational needs of customers. Enhances workflow through effective, open and respectful communication. Establishes and maintains open communication channels between pharmacy department and Legacy clinic. Demonstrates good listening skills. Negotiates and maintains work effectiveness with timely flow of information. Maintains strong rapport with health care practitioners. SAFETY AND SECURITY
: Patient, personnel and facility safety and security are ensured through knowledge and application of established policies and procedures and safety, security and emergency plans. Maintains certification and demonstrates knowledge of required safety, security and emergency programs required by department or hospital. Maintains clean, safe and orderly environment.
Qualifications
Education: Graduate of School of Pharmacy. Advanced degree (Doctor of Pharmacy preferred) with residency training or equivalent work experience required.
