Job Summary:
Responsible for the daily operations in the NHS care areas under the direction of the Senior Manager of NHS. Directly responsible for the Case Management teams and additional patient services areas and works closely with NHS providers and the Senior manager to set standards of clinical quality, productivity, safety measures in support of our SNF management providers. Supports the daily operational needs of the department as it relates to the patient care in the sub-acute and long-term care environments. Assures the delivery of quality patient care in a way that meets or exceeds standards while providing an excellent patient experience across the post-acute line of care.
Essential Responsibilities:
Operational Excellence: Manages the daily operations of assigned members requiring sub-acute and long-term care within the continuing care department to assure optimal delivery of safe, appropriate, accessible care and service. Supports/facilitates coordination of workflow across our care continuum in partnership with our NHS leadership team and supports care coordination efforts in our contracted and non-contracted SNF facilities. Collaborates as well with managers and CMLNs in the medical centers to align operations and implement/monitor interdisciplinary change initiatives, including access, workflow, quality, service, and safety improvements. Ensures that the IQM and CCQM quality initiatives are closely implemented and executed as in collaboration with our line of service line goals to support optimal care in our NHS environments. Assures adequate staffing levels are in place to meet patient needs in the post-acute environment. Patient Experience: Assures that staff deliver patient care that meets or exceeds clinical quality and safety standards. Trains and supports staff in delivering a consistent customer experience. Resolves service/care issues, unusual occurences (UOs), including patient complaints, as they occur within assigned area. Staff Engagement & Development: Manages staff, including selection, training and development, performance management, recognition, discipline, and termination. Supports all team members, including additional NHS staff, in achieving performance expectations and standards. Supports a culture of excellent service and communication that positively influences staff engagement. Assures a safe, productive, and supportive work environment for all staff. Financial Stewardship: Assists with managing the budgets for care management, identifies innovative and integrated ways to for optimal performance in support and direction from the Senior Manager. Manages within resources allocated. In collaboration with other medical center management colleagues, makes operational adjustments as needed to meet financial targets. Compliance and Emergency Preparedness: Establishes processes to monitor and support frontline staff in maintaining operational standards and practice in accordance with local, state, and federal requirements. Assures ongoing compliance and adherence to KPWA policy/procedure, as well as other compliance, regulatory and accreditation requirements. Assures that staff within the clinical team are trained and know what is expected during and after an emergency. Basic Qualifications: Experience
Minimum two (2) years of Case Management experience as a nurse either in the subacute environment or hospital. Minimum one (1) year of experience as a charge nurse, lead, or supervisor in a clinical environment managing the day-to-day work flow and performance of others, identifying and meeting customer requirements, and working effectively with medical staff. Education
Bachelors degree in nursing. License, Certification, Registration Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire Additional Requirements:
Ability to mentor, develop and engage staff, monitor performance standards, deliver results, and hold staff accountable. Ability to manage multiple tasks simultaneously. Effective communication, collaboration and change management skills. Current knowledge of subacute, SNF, and long-term care regulations and applicable state and federal regulations. Preferred Qualifications:
Two (2) years of experience as clinical lead, or supervisor in a long-term care environment managing the day-to-day workflow and performance of others, identifying, and meeting customer requirements, and working effectively with medical staff. Three (3) years of increasingly responsible operations experience as a supervisor, lead roles and or lead case manager. Knowledge of regulatory requirements, managed care, health care delivery systems and nursing practice.
