TravelNurseSource is working with MedCadre to find a qualified Case Manager RN in White Plains, New York, 10601 Pay Information $3,150 to $3,300 per week About The Position Job Title : Registered Nurse (RN) - Care Management Location : White Plains, NY 10601 Unit : Care Management, Unit Type : Case Management (Utilization Review) Division : Nursing Care Setting : Acute Shift : Days (10-hour shifts, 8 AM - 6 PM) Guaranteed Hours : 38 hours per week Job Start Date : Dec 9/Dec 16, 2024 Duration : 13 weeks Job Summary: Seeking a Registered Nurse (RN) for the Care Management Department. The RN will be responsible for performing comprehensive assessments, ensuring proper discharge planning, and working with the healthcare team to prevent readmissions and improve patient outcomes. The RN will also handle Utilization Review (UR) activities as per department policies. Responsibilities: Comprehensive Assessment : Conduct initial assessments on patient admission, utilizing tools to identify high-risk indicators and ensure appropriate discharge planning. Multidisciplinary Care Coordination : Coordinate care within the hospital and across hospitals, ensuring effective communication and collaboration with the healthcare team. Discharge Planning : Manage patient education, arrange aftercare, engage with commercial payers, and address high-utilizer patient root causes. Collaborative Communication : Ensure relevant patient information is communicated to appropriate team members, and participate in multidisciplinary rounds. Quality Improvement : Participate in performance improvement initiatives as needed. Patient Care for Geriatrics : Ensure safe discharge and continuity of care for geriatric patients, including inpatients, emergency department, and surgical outpatients. Other Duties : Perform other related duties as assigned. Required Qualifications: License : Valid RN license in New York State (NYS). Experience : 2 years of clinical experience required. Experience in case management, including discharge planning, is preferred. Prior experience in ED, critical care, or med/surg is preferred. Familiarity with Milliman/InterQual criteria is preferred. Certifications : RN License of NY State BLS required. COVID-19 vaccination required. PRI Certification. Care Management certification (CMC, CCM, etc.) is preferred. Skills : Strong computer and documentation skills. Knowledge of healthcare financials, regulatory, and payer issues is preferred. Familiarity with state, local, and federal programs is a plus. This role requires a proactive, detail-oriented RN with a strong background in patient care and case management to help optimize patient outcomes and ensure safe, efficient discharge planning.
