TravelNurseSource is working with Accede Solutions Inc to find a qualified Case Manager RN in Eureka, California, 95534 About The Position Registered Nurse/Case Manager Duration:13 Weeks Start: 01/06/2025 Shift : Day 5x8-Hour (08:00 - 16:30) 100% Onsite Requirements The RN Case Manager acts as the coordinator of the health care team for individual patients in order to maintain the proper linkages within a continuum of care for best patient outcomes. The RN Case Manager administers medication and treatments, maintains and submits the clinical record for each patient and provides progress notes, as needed.
